Ensure that the Export Value in your PDF Checkbox matches EXACTLY the Label value from the XForm. This can be found under the 'Options tab' in Adobe.
Ensure that the Name value for your PDF matches EXACTLY the Label Value for your XForm.
For a Radio button ensure you are using the 'Radio Button' field type in the PDF. Then, ensure that the question text label on the XForm matches the Name field in the PDF. This does not have to match exactly.
Next, ensure that the Radio Button Choice on each Radio option matches the options you have created on your XForm. You will map this only one time but need to have as many different radio button options in your PDF as you have in your XForm.
